## Introduction

A headline says “new bipolar medication.” That sounds straightforward until you realize that bipolar treatment can mean treating very different kinds of episodes.

Before asking whether a new medication is right for you, ask a more useful question: What exactly is it intended to treat?

## What received approval?

Bysanti, or milsaperidone, received FDA approval on February 20, 2026. Its uses include schizophrenia and the acute treatment of manic or mixed episodes associated with bipolar I disorder in adults.[1,2]

That is a specific approval. It does not establish it as a treatment for bipolar depression, ordinary mood changes, or every phase of bipolar illness.[2]

“Acute” refers to treating an active episode. If a medication is being considered during a crisis, ask separately what the longer-term plan will be once that episode improves.

## Mania can be harder to recognize than the stereotype

The American Psychiatric Association describes mania as potentially involving unusually elevated or irritable mood, increased energy, less need for sleep, racing thoughts, and risky or impulsive behavior. Mixed features can involve depressive and manic symptoms together.[3]

Less need for sleep is especially worth describing carefully. Feeling exhausted because you cannot sleep is a different experience from sleeping very little while feeling unusually energized.[3]

Rather than trying to diagnose yourself, bring a timeline: sleep, energy, mood, spending, behavior, medication changes, and what people close to you noticed. Specific examples give your clinician more to work with than “I was all over the place.”

## Why does this medication start gradually?

The prescribing information says:

“Titrate BYSANTI to reduce the risk of orthostatic hypotension.”[2, section 2.1]

That means increasing it in steps to reduce the chance of blood pressure dropping when you stand up. Bysanti is taken twice daily. The label also has a restart schedule after more than three missed days; contact the prescriber rather than resuming on your own.[2]

Other considerations include heart rhythm risk, medication interactions, metabolic changes, and abnormal movements. Its boxed warning addresses increased mortality in older adults with dementia-related psychosis, a condition for which it is not approved.[2]

## New is a reason to ask questions

It is not, by itself, a reason to replace a treatment that is working for you. Bring practical questions:

If there is dangerous behavior, severe confusion, or inability to remain safe, seek urgent assessment. A medication article cannot determine whether an active episode can be managed safely through routine outpatient care.
