## A practical starting point

When you are depressed, “give it more time” can feel like being put on hold by your own brain. Research on earlier improvement matters. So does understanding what researchers mean by “earlier.”

Auvelity combines dextromethorphan and bupropion. It was approved for major depressive disorder in adults in 2022.[1] You may recognize dextromethorphan from cough products, but that does not make a cough medicine a substitute for this prescription treatment. Do not attempt to recreate it yourself.

## What did “one week” mean in the study?

The published GEMINI trial reported improvement compared with placebo:

“starting 1 week after treatment initiation”[2, Conclusions]

That was an average difference on a depression rating scale. It was not a promise that every participant felt well within seven days.

In this six-week study of 327 adults, 39.5% receiving dextromethorphan-bupropion reached the study’s remission threshold, compared with 17.3% receiving placebo. Remission meant a low symptom score at that assessment, rather than proof that depression could never return.[2]

The study compared the medication with placebo. It does not establish that Auvelity is better than every commonly prescribed antidepressant, and its results do not apply automatically to everyone with depression.[2]

## Why is it in the news again in 2026?

On April 30, 2026, the FDA approved an additional use: agitation associated with dementia due to Alzheimer’s disease in adults.[1] That is a separate condition. It does not make Auvelity a general treatment for irritability, stress, or ordinary frustration.

If you see a new headline, ask which diagnosis the news actually concerns. The same brand name can appear in very different treatment conversations.

## The medication list matters

Before treatment, the prescriber should review blood pressure, bipolar history, and other products containing bupropion or dextromethorphan. Important risks include seizures, increased blood pressure, activation of mania, and serotonin syndrome with certain combinations. A seizure disorder or current or past anorexia or bulimia can rule it out.[3]

Common side effects include dizziness, headache, diarrhea, sleepiness, dry mouth, sweating, and sexual dysfunction. Antidepressant labeling warns about increased suicidal thoughts and behaviors in younger patients and calls for monitoring.[3]

Tell your clinician about cough medicines, supplements, and alcohol use—not just your psychiatric prescriptions.

## Give follow-up something concrete to measure

Before starting any new depression treatment, write down two or three changes that would matter to you. Examples might be getting out of bed with less struggle, enjoying dinner with your family, or completing a workday with less distress.

Then ask: When should we look for change? What should I report sooner? If there is some benefit but troublesome dizziness, what are our options? What will this cost after any introductory discount ends?

“Faster” is useful information. Your ability to function, tolerate treatment, and afford continued care still belongs at the center of the decision.
